Employee Evaluation Form
This Employee Evaluation Form is simply a template that allows you to provide a formal evaluation record to your employees following a performance review. It allows both employer and employee to keep a record of what was spoken about and forms the basis of an ongoing evaluation of performance that can be cross referenced at later reviews. It is designed to be customisable in that the employer can edit the categories for review dependent on their industry and the skills and traits they are seeking from their staff. The categories below are given simply as a guide to the general things an employer is look for in his or her staff.

Basis of Review |
The purpose of this evaluation is to provide the Employee with an indication of their progress within the job role and to highlight areas at which they excel and also areas where the Evaluator believes they can improve.
The Evaluator has been given a scale on which to rate the Employee’s performance in a number of different areas as follows:
Unsatisfactory, failing to meet expectations Fair, sometimes fails to meet expectations Satisfactory Good, frequently exceeds expectations Excellent, exceeds expectations

Does the Employee has a positive attitude towards their employment and do they approach their job and assigned tasks with the appropriate willingness and diligence?
